How to Ask the Court to Enter Orders: When You Agree

Please use this check list to ensure that you submit all of the necessary orders to the court for completion of your case (by agreement).

Divorce & Separation

DECREE OF DISSOLUTION OR SEPARATION

If you have agreed on all of the issues required for the court to grant a divorce or legal separation, you may use this form to ask the court to make an official order regarding your case.

PROPERTY SETTLEMENT AGREEMENT

Agreements about how assets and debts are divided can be entered into this form and submitted to the court to be entered as an official order.

Custody

PARENTING PLAN ORDER TO BE ENTERED BY THE COURT

If you and the other party have FULL agreement on all issues related to custody such as communication, decision-making, parenting time, etc, you can submit your agreements using this form so that the court can make them an official order.

JOINT LEGAL DECISION-MAKING EDUCATION ORDER

In Arizona, education orders are a part of family law that help schools understand which parents have the legal right and responsibility for their child’s education. Education orders can address issues such as: 
  • Decision-making: Who has the legal authority to make decisions about the child’s education
  • Pick-up: Who is allowed to pick up the child from school
  • Emergency contacts: Who to contact in an emergency
  • School access: Who is permitted to access school grounds, events, and activities
  • Permission slips: Who can sign permission slips and authorize extracurricular activities
  • School records: Who can access school records and the parent portal
  • Drop-off: Which parent is responsible for dropping the child off at school
  • Parenting time: Which parent has parenting time and should pick up the child from school

Education orders are binding on the parents but not on the school. They are intended to supplement the court-ordered parenting plan and reduce the involvement of teachers and school officials in disputes between parents.

Child Support

REQUESTING YOUR CHILD SUPPORT ORDER BE ENTERED BY THE COURT

This order can be submitted to allow the court to enter your agreements regarding child support-related issues as an official order of the court.

Temporary & Partial Orders

TEMPORARY OR PARTIAL ORDERS TO BE ENTERED

If you and the other party have come to an agreement, you can ask the court to enter those agreements as orders. Use this form if the agreements DO NOT resolve all of the issues in the case or if the agreements are ONLY temporary.
